What is in this vial
It is the smaller of the Follistatin 344 vials sold here, which means less substance standing in solution at a time. The powder has no concentration of its own: it is set when you add the water. This vial with 1 ml of bacteriostatic water gives 1 mg per millilitre, and with twice that volume, half as much.

What the evidence says

The history of the clinical trials
ACE-031 was studied in boys with Duchenne muscular dystrophy, where blocking myostatin was meant to support muscle growth and repair. Acceleron Pharma and Shire halted those trials on 21 April 2011, and on 2 May 2013 they closed the whole programme without resuming it. The reason was safety: some participants had nose and gum bleeding and small dilated blood vessels in the skin, which resolved after stopping. They were linked to blocking BMP9 and BMP10 signals through the activin receptor, which matter for normal blood vessel function.
For a buyer this means the mechanism was tested in clinical conditions and showed both an effect on muscle and an unexpected side effect. We do not write that ACE-031 builds muscle in a proven and safe way: for healthy people there are no clinical data confirming it.

What follistatin does
Myostatin, also labelled GDF-8, is a protein of the TGF-beta superfamily that restrains the growth of skeletal muscle. Animals and people with a mutation that switches off the myostatin gene have clearly more muscle mass, and that observation is the basis of the whole idea of blocking this protein pharmacologically. Follistatin is a secreted protein that binds ligands of the TGF-beta superfamily, myostatin among them, and so lifts that restraint. The number 344 in the name refers to one of the variants of the molecule.
Where the impressive figures come from
The results most often used to advertise follistatin come from mice without the myostatin gene. Those animals have a muscle physiology different from animals and people with a normal gene, so the results cannot be carried over to humans directly. There are no randomised human trials confirming muscle gain after follistatin, and follistatin 344 has not been approved for any indication.
This pack in numbers
The Genheal box holds one 1 ml solvent ampoule. One milligram of protein and one millilitre of solvent give 1000 mcg per millilitre, so on a 1 ml insulin syringe one unit equals 10 mcg. Add the solvent slowly down the wall of the vial, never straight onto the powder. Do not shake the vial, and keep the finished solution in the fridge at 2 to 8 degrees Celsius.

Ready-made protein, not a gene: a 2009 study
Most studies of follistatin 344 gave animals or people a gene that then made the protein in the muscle. The vial contains something different: the ready-made follistatin molecule as a lyophilised powder. That is why the study closest to this product is a 2009 one from the University of Missouri, in which exactly the ready-made protein was given.

The study was done in mice with spinal muscular atrophy, a severe disease in which the nerve cells controlling the muscles die. It showed that the protein given from outside is enough for the effect, without gene therapy.
Why the 344 variant
The follistatin gene gives rise to two main variants that differ in chain length. The Ohio team chose the 344 variant for its studies on purpose. It circulates in the blood and binds poorly to other cells, so it acts mainly on skeletal muscle. In a 2009 review, the same team noted that the 344 variant did not change fertility in animals.
