Tirzepatide is a dual agonist: it stimulates the GLP-1 receptor and the GIP receptor at the same time. In registered form it is a prescription medicine; what is sold here is research material, not a medicine.

What is in the vial
Genheal is the only manufacturer offering tirzepatide here, in a 20 mg vial. Set that against the registered medicine, where the highest weekly dose in the weight trials was 15 mg, and the scale of a single vial becomes clear. A 20 mg vial with 2 ml of bacteriostatic water gives 10 mg per millilitre, which means very small volumes per administration and a real risk of arithmetic errors: see reconstitution and syringe units.
What the trials showed
In SURMOUNT-1, tirzepatide at 15 mg reduced body weight by 20.9 percent over 72 weeks, against 3.1 percent on placebo. In SURMOUNT-5, the only head-to-head comparison in this group, it reached 20.2 percent against 13.7 percent for semaglutide over 72 weeks, with waist circumference falling 18.4 cm against 13.0 cm.
Fewer people stopped treatment because of gastrointestinal symptoms on tirzepatide than on semaglutide in that trial: 2.7 percent against 5.6 percent.
Why the second receptor
GIP receptor stimulation adds appetite-suppressing neural circuits and thermogenesis to what GLP-1 already does. Curiously, blocking the same receptor also reduces body weight, by a different route; both directions are described in the incretin guide and in more detail in why tirzepatide acts on two receptors.
Status
Approved by the FDA in May 2022, sold as Mounjaro for type 2 diabetes and Zepbound for obesity. In Poland the registered product is Mounjaro, on prescription. Research material is not that product.
