What is in this vial
Genheal fills tirzepatide as powder in a 20 mg vial, with one solvent ampoule in the box. The powder has no concentration of its own: it is set when you add the water. This vial with 2 ml of bacteriostatic water gives 10 mg per millilitre, and with twice that volume, half as much.

Genheal as the manufacturer

What the evidence says
In SURMOUNT-1 at 15 mg it reduced body weight by 20.9 percent over 72 weeks against 3.1 percent on placebo, and it beat semaglutide in the only head-to-head trial. Twenty milligrams in one millilitre
The Genheal box holds one solvent ampoule of one millilitre. Dissolving the whole vial in it gives 20 mg of tirzepatide per millilitre. A U-100 insulin syringe holds one hundred units per millilitre, so at this concentration one mark on the scale equals 0.2 mg of substance, or 200 micrograms.
The most costly mistake with this vial: a unit on an insulin syringe is not a millilitre. Drawing half a millilitre at this concentration gives 10 mg at once, fifty marks instead of a dozen or so. At 20 mg per millilitre the margin for such a mistake is narrower than with a 3 or 5 mg vial, and the conversion is worked through in the guide on syringe units.

Bio-Peptide gives two ampoules and so a finer scale. The Nouveaux pen removes reconstitution and works out cheapest per milligram. This vial is the choice for the simplest set-up: one vial, one ampoule, one concentration.
Powder and ready solution
A closed vial of powder tolerates room temperature much better than a solution. Once dissolved, time starts to count, because a peptide in water is less stable. What this vial is not
Tirzepatide is the active substance of the registered medicine Mounjaro, dispensed on prescription. The content of this vial is research material: the same molecule, a different legal status and different batch quality control. A research label does not change the legal status of the substance and gives no guarantee of the kind a medicine carries. Tirzepatide and blood pressure: what studies showed
Tirzepatide is the active substance of the drug Mounjaro. The numbers below come from clinical studies of that drug, run under medical supervision. The research material in this vial is not the drug and has neither its label nor its quality control.
The effect on blood pressure was measured most precisely with a 24-hour monitor in 600 participants of the SURMOUNT-1 trial. The monitor measured blood pressure every 30 minutes during the day and every hour at night, at the start and after 36 weeks.

The fall was visible by day and by night, regardless of age, sex and BMI. After 72 weeks, 58.0 percent of people on tirzepatide and 35.2 percent on placebo had normal blood pressure. Body weight explained about 70 percent of the fall in pressure, so the rest comes from other routes.
Strongest where blood pressure was high
An analysis of five SURPASS trials in people with type 2 diabetes showed an important pattern. The largest fall was seen in people with a systolic pressure above 140 mm Hg. In people with a pressure below 122 mm Hg there was no further fall. The authors write that this pattern eases concerns about blood pressure dropping too low.
Episodes of low blood pressure did occur, though rarely, and more often at higher doses. People with low blood pressure or taking medication for high blood pressure should measure it regularly.
