What is in this vial
Retatrutide comes here in two 20 mg vials, from Bio-Peptide and Genheal, and in a 40 mg ready-to-use pen from Nouveaux Ltd. Between the two vials the choice is a choice of manufacturer, not of quantity. The powder has no concentration of its own: it is set when you add the water. This vial with 2 ml of bacteriostatic water gives 10 mg per millilitre, and with twice that volume, half as much.

TRIUMPH-1 gave 25.0 percent weight reduction at 12 mg after 80 weeks, and 30.3 percent at 104 weeks in the subgroup with a body mass index of 35 or above. Retatrutide has finished phase 3 trials and is approved nowhere, which is an unusual combination: more published human data behind it than most of this catalogue, and no regulator's decision. Bio-Peptide and Genheal both fill 20 mg, against a highest trial dose of 12 mg per week.

How fast body weight fell in the phase 2 trial
The first large results for retatrutide came from a phase 2 trial published in 2023 in the New England Journal of Medicine. It included 338 adults with obesity, or with overweight and a related condition. The drug was given under the skin once a week for 48 weeks. Body weight was measured after six months and after a year.

At 8 and 12 mg the result after six months was larger than the result after a year at 1 mg. In the 12 mg group, every participant who stayed to week 48 lost at least 5 percent of body weight. At least 15 percent was lost by 83 percent of people in that group, against 2 percent on placebo.
The doses in the table are the doses studied in the protocol, not a recommendation for research material. The most common side effects concerned the digestive tract and rose with the dose. The researchers also described a rise in heart rate, largest at week 24.
Results in people with type 2 diabetes
People with type 2 diabetes lost weight more slowly than people without diabetes, though still clearly. The same pattern is known from studies of semaglutide and tirzepatide.
- Phase 2, 36 weeks: at 12 mg, weight loss of 16.94 percent against 3.00 percent on placebo.
- TRANSCEND-T2D-1, 40 weeks: 537 people who had had diabetes for 2.5 years on average, at 12 mg a loss of 15.3 percent against 2.6 percent.
- TRIUMPH-2, 80 weeks: 1152 people with obesity and diabetes, at 12 mg a loss of 18.8 percent against 5.1 percent.
